Don’t over think this and ramble folks. Literally follow the directions. Y’all are too smart for your own good and asking deep legal questions but skipping the basics. Here’s the how to’s, and it’s simple, so keep it there :).
Important side note: there have never been nor will there ever be any legal prescriptions of marijuana so don’t ask for it. Prescription is the guaranteed way to get your doc to say no. Drop the word prescription from your vocabulary. These are certifications. The difference is tremendous. One ends with the doctor going to prison, the other ends with you getting better.
Legally speaking there is no reason for your doctor to not sign paperwork to certify whether you have a condition or not. They have nothing to do with dispensing and are fully protected under state AND federal law. Iowa has spent ten years, and I’ve been there every step of the way, to become the first state in the country to say we have resolved federal law, so don’t think we’re like the other states. In that regard, we are better.

Who Can Submit a Petition?
Any person or entity may file a petition to add medical conditions with the Medical Cannabidiol Advisory Board.
The petition form may be filled out electronically or hand-written, and then completed with a signature and date. The petition may be emailed, mailed or hand delivered to:
Please Be Aware
- Only fill out your petition for a single condition.
- Completely fill out each section that applies to your petition. Failure to do so may result in a denial of consideration by the board.
- The board may request that you submit additional information concerning your petition, and will notify you of the requested materials if more information is needed.
- At the next scheduled advisory board meeting, the board may submit this petition for a public comment period. If you submit a petition, it is in your best interest to be present when the board is deliberating over your petition.
- The board has six months after you submit this form to deny or approve the petition. If approved, you will be notified of their recommendation to the board of medicine. If denied, you will be notified of the reason for denial.

Adult Patient Registration
All adult patients age 18 and over must complete a patient registration application. Minor patients (under 18 years old) and any adult patients who need a caregiver will need the caregiver to complete a separate caregiver registration application (see caregiver registration tab on the left side of this screen). Registration cards are valid for one year and must be renewed annually.

3. Submit your application. All applications must include:

- Completed application
- Heathcare practitioner certification form signed by your doctor
- Copy of your photo ID which can be either:
- A valid Iowa driver’s license, OR
- A valid Iowa non-operator’s identification card
- Copy of proof that you qualify for the reduced fee (if applicable) which can be any of the following:
- Copy of Social Security Disability Benefit notice (SSDI notice)
- Copy of proof of receipt of Supplemental Security Income Payment (SSI receipt)
- Copy of Iowa Medicaid member card
- Copy of Hawk-I member card
- Payment
- $100 for patient registration cards unless you provide proof that you qualify for the reduced fee (see above)
- $25 for reduced fee patient registration cards
- $25 for caregiver registration cards

4. Next Steps

- The Department of Public Health will review your application and tell you if it is approved or denied
- If your application is approved, you will need to visit the closest driver’s license station to get your registration card
- Visit any of Iowa’s licensed dispensaries to purchase products
