Instructions For Filing Petition To Add Qualifying Conditions To The Medical Cannabis Program

 

Who Can Submit a Petition?

 

Any person or entity may file a petition to add medical conditions with the Medical Cannabidiol Advisory Board.

 

The petition form may be filled out electronically or hand-written, and then completed with a signature and date. The petition may be emailed, mailed or hand delivered to:

Please Be Aware

 

Only fill out your petition for a single condition.

 

Completely fill out each section that applies to your petition. Failure to do so may result in a denial of consideration by the board.

 

The board may request that you submit additional information concerning your petition, and will notify you of the requested materials if more information is needed.

 

At the next scheduled advisory board meeting, the board may submit this petition for a public comment period. If you submit a petition, it is in your best interest to be present when the board is deliberating over your petition.

 

The board has six months after you submit this form to deny or approve the petition. If approved, you will be notified of their recommendation to the board of medicine. If denied, you will be notified of the reason for denial.
