Start with the expiration date on your card, then work through certification, application, payment, and delivery.

Your next step
Check your card’s expiration date and contact your certifying practitioner. Use DOH’s official portal rather than starting a second account.

1. Start before your card expires

The renewal rule allows submission up to 45 days before expiration. That is a renewal window—not a requirement to apply at least 45 days early. DOH’s reminder arrives 45 days before expiration and directs patients to arrange a new practitioner certification.1

A card can expire sooner than a full year: state law uses the expiration date in the practitioner’s certification, with a one-year maximum from issuance.2

2. Follow the application sequence

  • New patient: the practitioner creates the certification in the registry first; then activate the patient account and complete the application. Check your junk folder for the activation email.3
  • Renewing patient: after your practitioner renews the certification, sign in to the official patient portal with your existing credentials. Use its password-reset option if needed.4
  • Check the patient’s mailing address and contact information before submission. Keep a record of your confirmation and payment.

3. Gather documents and budget for the fee

Have an unexpired ID and an acceptable patient photo ready. If seeking the low-income rate, gather the income or benefit documents requested by the program. The standard patient application or renewal fee is $75; the low-income rate is $20. Practitioner charges are separate—ask the practice what its visit costs.5

Ask DOH which caregiver or cultivation fees apply to your transaction. The October 26 Register lists amendments to the fee rule as effective November 5, 2026. A dated instructional PDF should not be treated as the final word on an amended fee exception.6

4. Confirm submission and watch the status

DOH’s renewal tips warn that changing a submitted application moves it to the bottom of the review list. Check carefully first; if an error needs correction, ask the program how to fix it. Approval and arrival of the physical card are separate steps.7

If time is running out
Contact DOH about your application status and ask the dispensary what it needs to verify a current card. A pending application or an approval email should not be assumed to extend an expired card. Ask your clinician about continuity of care if access may be interrupted.

5. Keep a simple renewal record

  • Card expiration date; practitioner appointment date; certification date.
  • Application submission and payment confirmation.
  • Any request for missing documents and your response.
  • Approval date and date the replacement card arrives.

For portal or card-status help, use DOH’s current patient contact page. If a denial or nonrenewal notice arrives, keep the entire notice and promptly seek advice about review options and deadlines. The statutes provide notice and judicial-review provisions; a customer-service inquiry is not a substitute for protecting a review deadline.8
